Exam Duty Application

Declaration / Affidavit I certify that: i) None of my son, daughter, brother, sister, or any relative is appearing in the examination at this examination centre. ii) I have not been declared disqualified by any Board/University for any type of duty. iii) In the area related to this examination centre, no candidate (male/female) who is appearing in the examination is living with me or has taken tuition from me. After giving my consent, I will not excuse myself, and I will keep every matter related to my duty strictly confidential. During the examination, if any close relative appears for the examination at the relevant centre, I will immediately inform the office and will leave my duty at once. I undertake that I will perform my duties with complete honesty and confidentiality. During the examination, I will not accept food, etc., from any candidate (male/female) or from the institution’s staff.
